Frequently Asked Questions About Birth Support in Utah & Salt Lake Counties
A CPM works with low-risk clients out of the hospital setting. CPMs don’t have rights at hospitals, and primarily offer care at birth centers and in home settings. As your CPMs we have undergone schooling at a MEAC accredited school (Midwives College of Utah) and received our bachelors degrees there. We have also had an apprenticeship for 2–6 years before sitting for the NARM and becoming licensed. CNMs are midwives that primarily work in the hospital, though there are a few who do offer out of hospital birth. CNMs go to nursing school first and then add onto their nursing degree with a midwifery program after that.
One of the biggest benefits of hiring us as your midwives is that we do ALL in home appointments. No waiting in stuffy offices, finding childcare, or having to load everyone in to the car. We come and meet you in your home.
Our appointment schedule is that of a typical OBGYN. We see you every month from 10–28 weeks gestation, then every 2 weeks from 28–36 weeks gestation, and weekly from 36 weeks until birth. One of the benefits of working with us is that you get access to our text and call line for urgent and emergency needs as well as non-urgent questions too. We also are happy to come and stop in outside of regularly scheduled appointments for urgent concerns and needs.
During your prenatal appointments we answer any questions you and your family may have, educate on prenatal nutrition and supplementation, help find solutions for pregnancy symptoms, go over vital signs, and listen to your baby with our doppler or fetoscope. We also ask our own set of questions that helps us to rule out any concerning pregnancy symptoms that you may have.
We often spend portions of the appointment just getting to know you and your family. We also love appointments that include the entire family and enjoy helping your kids feel a part of your care. It is common for kiddos to be helping us with your vitals and even with the Doppler listening to your baby. We understand that this is an experience that impacts the entire family and want everyone who wants to be included, to be included.As a certified professional midwife who is licensed in the state of Utah, we can carry specific medications. Some of the medications we carry include:
— Antihemorrhagic medications like Pitocin, Methergine, Misoprost, and TXA. These are the same medications used in hospital settings to help with excess bleeding postpartum.
— IV fluids. We utilize IV fluids to help with hydration and exhaustion during childbirth and to replace fluids when needed after a hemorrhage postpartum. We can also use IV fluids in pregnancy to help with kidney stones, UTIs, and morning sickness as needed.
— Antibiotics. We carry antibiotics for individuals who are GBS+ and wish to treat preventatively with IV antibiotics.
— Oxygen. We bring at least two oxygen tanks to every birth that we attend. Oxygen can be helpful for a birthing parent who is feeling like they are struggling to catch their breath during pushing, but more commonly, for a newborn that is needing extra help immediately postpartum. We are trained in neonatal resuscitation and can offer respiratory support like CPAP for up to 4 hours after birth as long as baby is responding to our efforts.As CPMs, we are professionally trained in low-risk pregnancy, birth, and postpartum. This means that we are also professionally trained to know when something is veering outside of low-risk and when it is time to transfer into a higher level of care. We are not the midwives who are homebirth at all cost, we are the kind of midwives who value homebirth but value a parent and baby’s life more. We prefer to transfer before things are crazy, and that means we are hypervigilantly looking at signs and symptoms throughout the entirety of your care.
If a true emergency occurs, we are calling EMTs and preparing you for what is next. One of us ALWAYS rides on the rig with you as we are better trained in birth emergencies than EMTs typically are. We then stay with you throughout the entirety of your birthing process and into postpartum to get you settled and, ideally, feeding and bonding with your baby. Yes, it is a change of location, but you get to keep us as a part of your birth team to help advocate and support you. When you get back home, we pick up with postpartum visits right where we left off.We continue with all in-home appointments, just like your prenatal care. Our visits are typically about 1 hour in length and during those appointments we spend time looking over you and your baby’s vitals, educating on nursing and feeding, answering any questions and concerns that may arise between visits, and processing your birth and postpartum experiences. We can offer all of your baby’s pediatrician care for the first 6 weeks postpartum as long as they stay low-risk, but after 6 weeks you need to be set up with your pediatrician and seeing them for any problems as needed.
Our visit schedule is that we come and see you at 24–36 hours postpartum, this is the appointment that we do the hearing screen, heart screen, heel poke, foot prints, and birth certificate at. Then we come and see you at 1 week, 2 weeks, 4 weeks, 6 weeks, 9 weeks, and 12 weeks postpartum. If you have needs outside of these scheduled visits, we can always pop in between them.Placenta encapsulation is when we take the placenta and steam it, dehydrate it, and then grind it up and put it into capsules to be ingested.
The placenta is a filter but not in the same way a cigarette filter works. Instead of collecting the gunk and holding onto it, the placenta filters any bad stuff back into the parent’s body for them to process and eliminate. Because of this, the placenta is not filled with toxins and other dangerous things for ingesting.
Anecdotally we almost always see placenta capsules help with mood and emotions. We have both partaken in our own placenta capsules postpartum and felt this benefit, and often see it with our clients who choose to ingest. Other anecdotal side effects we have witnessed are more energy and quicker healing post birth.
We like to find our way to the middle and balance modern medications with holistic approaches. In our practice, we find that most pregnancy, birth, and postpartum ailments can be solved first by diet and lifestyle change and second by supplementation, but we do recognize that there are times where these things are simply not enough to fix the problem at hand. This is where we honor modern medications and what they can do for us. As an Apothecary owner, I see the value in herbs and food as medicine and try to utilize these holistic approaches first when at all possible. It is also important to me that my clients know how much I value the modern medications we are able to bring in as needed, and that we won’t hesitate to utilize these tools if the more natural minded ones are simply not doing enough.
